GPS Watch: Garmin Forerunner 920XT (I use this for all events.)

Swimming:

Suit: SOAS Ultramarine Tri Tank & Shorts

Wetsuit: Roka Maverick Elite

Goggles: Aqua Sphere Kayenne Swim Goggles

Paddles: Finis Agility Hand Paddles

Flippers: Finis Yellow Zoomers

Pull Buoy: Finis Foam Pull Buoy

Cycling:

Bike: Cannondale Synapse Carbon 2014

Shoes: Louis Garneau

Cycling Shorts: SOAS Ultramarine Tri Shorts

Aerobars: Redshift Quick-Release Clip-on Aerobars

Running:

Shoes: Saucony Mirage 5

Misc:

Sunglasses: Tifosi

Fuel:
Huma Gel: I eat one 15 minutes before a long distance activity. I can't really stomach these while I'm out there but it's great, especially before the swim of a triathlon!

Cliff Shot Bloks: I start these about 30-40 minutes into a run and I eat one every mile or 10 minutes.

Heed: I've just started taking this on mid-distance bike rides. I still need a lot of practice drinking and eating on the bike but I like the all liquid nutrition method.
